DEFINITIONS  - A Guide to the Lingo

Accreditation
The Giving Tree is accredited by the Commission for the Accreditation of Rehabilitation Facilities (CARF). CARF is a national accrediting organization whose mission is to promote the quality, value, and optimal outcomes of services through an accreditation process based on standards that centers on enhancing the lives of the persons served. The Giving Tree received a three year accreditation which shows that its services and practices are designed and implemented to benefit the persons it serves. 

Assessment/Evaluation
The clinical assessment/evaluation consists of gathering information to assess client needs and functioning in order to determine appropriate service/treatment based on the identification of the presenting problem, evaluation of mental status, and formulations of a diagnostic impression. The outcome of the assessment/evaluation is to determine the need for care, and recommend appropriate services/treatment and/or the need for further assessment. An assessment must be completed prior to the initiation of any mental health or alcohol/drug addiction services.

Certification
Certification means the written authorization from the Ohio Department of Mental Health and the Ohio Department of Alcohol and Drug Addiction Services for an agency to operate specific services and provide activities according to the Ohio Revised Code. The Giving Tree is certified by both the Ohio Department of Mental Health (since 1984) and the Ohio Department of Alcohol and Drug Addiction Services (since 1996).

Community Psychiatric Supportive Treatment (CPST)
This service is also sometimes referred to as “case management”. CPST services provide goal-oriented and individualized supports focusing on improved self-sufficiency for the person served through assessment, planning, assistance, linkage, advocacy, coordination, education/training, monitoring activities and may provide coordination and/or assistance in crisis management and stabilization as needed.

Consultation Services
Consultation is time limited and initiated by the consultee or the consultee’s organization for the purpose of invoking the consultant’s expertise in understanding and managing the mental health aspects of a client or group of clients. The client is a client of the consultee's organization and not a client of the consultant’s organization. The consultant carries no treatment responsibilities for the client.

Cognitive Based Therapy
Cognitive-behavioral therapy is a relatively short-term focused therapy for a wide range of psychological problems including depression, anxiety, anger, marital conflict, loneliness, panic, fear, eating disorder, substance abuse, alcohol abuse and dependence and personality problems. The use of cognitive-behavioral therapy is used with children, adolescents and adults.
The focus of therapy is on how the individual is thinking, behaving, and communicating today rather than their early childhood experiences. The counselor assists the client in identifying specific distortions and the biases in their thinking and provides guidance on how to change this thinking.  Cognitive therapy helps the client learn effective self-help skills that can help change the way the client thinks, feels and behaves now. Cognitive-behavioral therapy is action–oriented, practical, and rational and helps the client gain independence and effectiveness in dealing with real-life issues. Cognitive–behavioral therapy has been proven to work based on scientific research and a large number of controlled trials on many different psychological conditions. It has frequently been shown to be more effective than other forms of therapy.  Research suggests it is currently the treatment of choice for many psychological conditions. For this reason cognitive-behavioral therapy is part of what is called “Evidence-Based Practice” i.e. it is firmly based on research.   

Counseling
Individual counseling involves an encounter between a client, or client and family members and a counselor. Individual counseling means the utilization of special skills to assist an individual in achieving identified treatment goals and objectives. Group counseling means the utilization of special skills to assist two or more individuals in achieving treatment objectives.  Group counseling helps because clients learn that they are not alone with their concern, learn new ways to resolve their problems, and learn about themselves and new ways of relating to others. Family Counseling involves the client and their family members and/or significant others and addresses family and relationship issues as well as problems that have developed in one or more family members which affect all. Skill building may include effective communication, conflict resolution, and assertiveness.

CPST Engagement
This service is for the purpose of engaging the resistant or homeless client in the mental health system. This service is limited to past adult severely mentally disabled eligible persons and to persons in public distress where the probability for severely mentally disability  eligibility is high. 
 
Crisis
Crisis means a situation with a high stress level for an individual and where usual coping methods do not succeed in resolving the issues presented.  
 
Crisis Intervention
Crisis intervention services are aimed at the assessment and immediate stabilization of acute symptoms of mental illness, alcohol and other drug abuse, and emotional distress or in response to acts of domestic violence or abuse/neglect. It may also include a determination of level of care in the least restrictive environment in a manner that is timely, responsive, and therapeutic. 
 
Custody and/or Visitation Evaluations
This may include pre-divorce custody, post-divorce change of custody, visitation/termination of visitation disputes or divorce mediation/counseling.  The services may include interviews and testing of parents, step-parents, and children, other involved parties, as well as collateral data review, interpretation, report preparation and testimony if required. This service is provided by a licensed clinical psychologist.  This is not a Medicaid billable service. Insurance may or may not cover this type of evaluation.

 
Employment/Vocational Service
The purpose and intent of this service is to promote recovery and secure/maintain employment by providing training and skills development that is goal-oriented, ability–based and incorporate individual choice. The outcomes of this service are that the client will obtain and maintain employment, learn new job skills, increase self-sufficiency and contribute to the community. Adult educational services are structured educational intervention for adults, such as educational advising, literacy instruction, basic educational instruction or instruction in community and independent living skills.  Adult educational services are provided if existing community educational programs are not available.
 
Evidenced Based Practices
Evidenced-based practices refer to programs or practices that are proven to be successful through research methodology and have produced consistently positive patterns of results. Evidenced–based practices or programs that have shown the greatest levels of effectiveness are those that have established generalizability (replicated in different settings and with different populations over time) through research studies.  
 
Forensic Evaluation Service
A forensic evaluation services means an evaluation resulting in a written expert opinion regarding a legal issue for an individual referred by a criminal court, domestic relations court, juvenile court, adult parole authority, or other agency of the criminal justice system or an ODMH behavioral healthcare organization.  A forensic evaluation is undertaken only with an appropriate order from the court ordering the evaluation or an official written request if the agency requesting the forensic evaluation is a parole or probation department or ODMH behavioral healthcare organization.

Hotline
This service is a twenty-four hour per day, seven days per week response to a telephone call, often anonymous, made by an individual for crisis assistance.

Intervention
Services, especially for at-risk youth, that seek to reduce alcohol and/or other drug problems and to intervene in such a way as to arrest the progression of such problems.  It includes early intervention services.

Medication
Medication services are also referred to as medical/somatic services, pharmacologic management services, and pharmacotherapy. This service addresses the psychiatric/mental health needs of a client, evaluates the medication prescription, administration, monitoring and supervision, informs clients and family regarding medication and its actions, effects and side effects so that they can effectively participate in decisions concerning medication that is administered/dispensed to them, assists clients in obtaining prescribed medications, when needed and provides follow-up.  Medication services are in response to specific symptoms, behaviors and conditions for which the use of medications is indicated and effective such as reducing/stabilizing and/or eliminating psychiatric symptoms with the goal of improved functioning including management and reduction of symptoms. Prescription, administration and supervision of medication are governed by professional licensure standards of the Ohio Revised Code. Staff includes licensed psychiatrists, registered nurses or licensed practical nurses.

Play Therapy
Play therapy is a technique used for establishing communication and resolving problems with young children.  Children use play to communicate at their own level and at their own pace without feeling interrogated or threatened. Research supports the effectiveness of play therapy with children experiencing a wide variety of social, emotional, behavioral and learning problems, including children whose problems are related to life stressors, such as divorce, death, relocation, hospitalization, chronic illness, stressful experiences, physical and sexual abuse, domestic violence and natural disasters.  

Prevention
(1) Alcohol and other drug prevention focuses on preventing the onset of alcohol and other drug (AOD) use, abuse and addiction.  AOD prevention includes addressing problems associated with AOD use and abuse up to, but NOT including assessment and treatment for substance abuse and dependence. Prevention activities are intended to inform, educate, develop skills, alter risk behaviors, and affect environmental factors and/or provide referrals to other services. (2) Mental health prevention is oriented either towards preventing or reducing the incidence, prevalence, or severity of specific types of mental disabilities or emotional disturbance. Prevention services may include competency skills building, stress management, self-esteem building, mental health promotion, life-style management and/or provide referrals to other services.

Psychological Evaluation
This type of evaluation is conducted by a licensed clinical psychologist and consists of a comprehensive evaluation of emotional, psychological and behavior issues of an individual.  A complete evaluation consists of clinical interview/s, testing interpretation, and compilation of the results into a written report. This is not a Medicaid billable service. Insurance may or may not cover this type of evaluation.

Recovery
Recovery is a personal process of overcoming the negative impact of a psychiatric disability despite its continued presence.  It is an interpersonal dynamic process of embracing hope, defining oneself, and participating in meaningful roles in the community.

Referral
This is a recommendation to seek or request services and/or evaluation between agencies in order to assess or meet the needs of persons served. 

SAP/DOT Assessments
This evaluation service is provided to an employee who has violated a Department of Transportation (DOT) drug and alcohol regulation.  The evaluation is conducted by a Substance Abuse Professional (SAP) who has the credentials and training to complete this type of evaluation.  This service consists of a complete face to face assessment and evaluation to determine the level of care required, appropriate recommendations including referral to in-patient or outpatient treatment programs, educational program, self-help, and/or community resource, follow-up monitoring to determine if the employee is demonstrating successful compliance with the original treatment recommendations, a face to face follow up evaluation with recommendations, a follow up testing plan and reports to the designated party. This is not a Medicaid billable service. Insurance may or may not cover this type of evaluation.

Treatment
Treatment begins with an assessment/evaluation of an individual. An assessment must be completed prior to the initiation of any mental health or alcohol/drug addiction services.  Services that may be recommended after the assessment/evaluation include individual counseling, family counseling, group counseling, CPST (see definition), pharmacologic management service (see definition Medication), and vocational or employment services.

Treatment Plan
The development of a treatment plan is based on an assessment/evaluation, a continuing assessment of needs, and the successful identification of services needed.  It is an individualized plan that is a collaborative process between the client and the service provider and contains goals and objectives that incorporate the unique strengths, needs, abilities and preferences of the person served as well as identified challenges and problems. The treatment plan also addresses a planned transition process.  Transition may include discharge, placement on inactive status, movement to a different level of service or intensity of contact. Transition services are critical for the support of the client’s ongoing recovery or well –being.

Urinalysis
Urinalysis means the testing of an individual’s urine specimen to detect the presence of alcohol and other drugs.  Urinalysis includes laboratory testing and/or urine dip screen.

Wrap Around Funds
Wrap around funds are for non-traditional services or supports to help alleviate the need for more restrictive treatment settings such as psychiatric inpatient setting and to aid in the timely resolution of crisis situations. For crisis situations, the intent of wrap-around funds is to enable greater flexibility and immediate access to resources to help stabilize the client’s life situation. The funds can also be used to augment traditional mental health services such as transportation to needed services, obtaining psychotropic medications on an emergency basis, or provide for obtaining emergency temporary housing.
