ASSESSMENT
Mental Health Assessment
service is a clinical evaluation provided by trained staff which includes gathering information to assess an adults needs and functioning in order to determine appropriate service/treatment based on the identification of the presenting problem, evaluation of mental status, and the formulation of a diagnostic impression. This is a fee based service. Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. 

CRISIS INTERVENTION
Crisis Intervention service is a face-to-face response with an adult and/or family members to a developing crisis situation and includes an evaluation of the presenting crisis, a risk assessment, mental status screening, immediate stabilization, and the determination of  the level of care needed. It may also include the coordination and referral for psychiatric hospitalization if needed. This is a fee based service. Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. INDIVIDUAL OR FAMILY COUNSELING
Individual and/or Family Counseling service focuses on the treatment of the adults mental illness or emotional disturbance. Family members, parents, guardians and significant others may also be involved in order to achieve the goal of improved functioning of the adult. Cognitive-behavioral therapy (CBT) is one form of therapy that is utilized by staff. Studies of CBT have demonstrated its usefulness for a wide variety of problems, including mood disorders, anxiety disorders, eating disorders, substance abuse disorders, personality disorders and psychotic disorders. Specialized services include iindividualized treatment for adults who are trying to overcome various forms of current or past abuse. This  may include incidents of domestic violence, sexual abuse, sexual assault or sexual harassment.  We do not recommend couples counseling for victims of domestic violence. This is a fee based service. Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. 

PHARMACOLOGIC MANAGEMENT (MEDICATION)
Pharmacologic Management service is provided by a psychiatrist, a medical physician who specializes in the diagnosis, treatment, and prevention of mental illnesses. The service includes a psychiatric and mental health examination, prescription for medication, education regarding the purpose, risk, side effects and benefits of the medication being prescribed, the administration and supervision of medication and follow up as clinically indicated. This is a fee based service. COMMUNITY PSYCHIATRIC SUPPORTIVE TREATMENT (Case Management)
Community Psychiatric Supportive Treatment provides goal-oriented and individualized supports focusing on improved self-sufficiency for adults either recovering from mental illness or coping with emotional distress. Services may include planning, linkage to services, advocacy, coordination, monitoring activities, management of basic needs, assistance in the development of daily living skills, access to natural support systems in the community, assistance in crisis management and family education and training.  There is no charge for this service. VOCATIONAL/EMPLOYMENT SERVICE (SEPALS) - SUPPORTIVE EMPLOYMENT- PREPARATION AND LINKAGES SERVICES
Vocational/Employment Services  provide training and skill development to adults recovering from mental illness with outcomes of obtaining or maintaining employment, learning new job skills, increasing self-sufficiency and contributing to the community. Our program, Supported Employment Preparation and Linkages Services (SEPALS) follows the philosophy that all choices and decisions about work and support are individualized, based on the persons preferences, strengths, abilities, and experiences and that employment is an important component of the recovery process. Our employment supports are not only individualized, but flexible and available for as long as necessary.  Our employment specialists work closely with service professionals and families to help individuals integrate successful job experiences into their lives. GROUPS

Purposeful Living is a counseling group for persons who are recovering from mental illness. Skillful ways of thinking and communicating are explored along with developing problem solving skills and coping skills. The group offers a safe place to talk about problems, build self-confidence, and  get support from others without shame. The group meets 90 minutes each week. This is a fee based service. Positive Solutions is a program for men and women who have a dual diagnosis of a mental illness and substance abuse (alcohol and/or drug abuse or dependency). Positive Solutions is called integrated treatment? because it combines treatment for both mental illness and substance abuse in the same program.  Research strongly indicates that to recover fully a person needs treatment for both problems. Focusing on one does not ensure the other will go way. Recovery themes explored in Positive Solutions include understanding mental illnesses,  roadblocks in recovery, medical and psychiatric effects of drugs and addiction, medication education, coping skills, family issues, changing negative thinking, and the recovery process.  Positive Solutions meets once a week for 90 minutes. Prior to participating in this program an assessment and a 1/2 hour of individual counseling is required. This is a fee based service. Anger Management for Substance Abuse and Mental Health Clients is a six-week cognitive behavioral anger management group therapy program consisting of 90-minute weekly sessions.  Cognitive behavioral therapy (CBT) has been found to be effective for anger problems. This treatment model is a combined CBT approach that employs relaxation, cognitive, and communication skills interventions.  This approach presents the participant with options that draw on these different interventions and then encourages them to develop individualized anger control plans.  Participants learn to identify triggering events, recognize the causes to anger, and develop anger management strategies in response to events and cues. Prior to participating in this program an assessment and a 1/2 hour of individual counseling is required.This is a fee based service. The Choices Program: How to Stop Hurting the People Who Love You provides treatment for domestic abuse offenders and focuses on ending verbal and physical abuse in intimate relationships. The goal of domestic violence treatment is to end the abuser?s violent and abusive behavior, increase victim safety, and to hold abuser accountable. The Choices Program is an effective tool that helps men and women master the concepts and skills needed to stop their destructive behavior, resolve problems appropriately, and maintain an abuse-free lifestyle. Program participants meet for 90 minutes weekly for 24 weeks. Individual therapy sessions are available as well. Please note that this is not an anger management program.  Anger management programs are not designed to address the fundamental causes of domestic violence or safety and accountability issues. Anger management is not an appropriate alternative to domestic violence treatment. Prior to participating in this program an assessment and a 1/2 hour of individual counseling is required.This is a fee based service. Sexual Abuse Treatment Program for Adults is an intensive treatment program for sexual offending adults 18 yrs. of age and older. The goals of the program are to protect the community and provide for public safety, to prevent or minimize the possibility of reoffense, and to teach participants to responsibly manage their own behavior.  Treatment consists of 90 minute weekly group sessions and 
individual and family sessions as needed.  Participation in the program is generally for two years. A special 60 minute monthly support and education group is provided at no charge for family members, partners, parents, and significant others of the offender. The Sexual Abuse Treatment Program works closely with probation/parole and other involved agencies to provide maximum community safety. This is, in part, accomplished through a monthly multi-disciplinary interagency team meeting. This is a fee based service. CONSULTATION
Consultation is a time limited service between agency staff with mental health expertise and a person or other agency or organization. The purpose is to obtain guidance in understanding and managing the mental health aspects of an identified adult.  TPO (TEMPORARY PROTECTION ORDER) ASSESSMENT 
Assessment for victims of domestic violence where the victim has suffered emotional, physical, and mental abuse. The results provide important risk and needs assessment.  It also facilitates an understanding of a victim's emotional and mental health needs and provides a basis for the recommendation of appropriate treatment or intervention, if needed.  The assessment consists of an interview and evaluation followed by a report that is issued to the referring agency or organization. SEX SPECIFIC EVALUATIONS
Sex Specific Evaluation is a very specific type of formal evaluation conducted with adult sexual offenders. A detailed written report with recommendations is completed following the evaluation.  This is a referral based only service and is a private pay service.  For more information contact Mary Kay Baumgartner at 419 734 2942 ext. 14.

PSYCHOLOGICAL EVALUATIONS
Psychological Evaluation consists of a comprehensive evaluation by a licensed clinical psychologist of emotional, psychological and behavioral issues of an adult.  A complete evaluation consists of clinical interview/s testing, interpretation, and compilation of the results into a written report. In order to receive an evaluation, a written referral must be received (court, attorney, agency, etc). The retainer fee for this service is $600.00. If the evaluation extends beyond four hours, the fee is $150.00 per additional hour. For more information contact Marty Williams at 419 734 2942 ext. 20.
 
CUSTODY AND/OR VISITATION EVALUATION SERVICE
Custody and/or Visitation Evaluation services includes: pre-divorce custody, post-divorce change of custody, visitation/termination of visitation disputes, divorce mediation/counseling.  These services may include interview/s and testing of parents, step-parents, and children, other involved parties, as well as collateral data review, interpretation, report preparation and testimony if required.  The retainer fees are $900 per adult, $600 per child (13 yrs. of age and older), $300 per child (12 yrs of age and younger), $600 for testimony.  Each adult involved in the service is required to pay the full adult retainer fee.  Each parent is required to pay a retainer fee equal to one-half of the children's total evaluation cost.  If services extend beyond the retainers, the fee is $150 per additional hour. SENSIBLE APPROACH
Sensible Approach is a required workshop by the Ottawa County Common Pleas Court for all parents with minor children who are parties to a divorce or dissolution. This workshop is provided as a collaborative effort between The Giving Tree and the Ottawa County Common Pleas Court.  The workshop focus is Children in the Middle - a two hour skills based program that teaches parents how to support and assist their children as they adapt to changing family relationships. The program was developed by experts in the field at the Center
for Divorce Education. Rigorous testing has shown the program to be not only effective but also popular with parents. The program consists of an introduction, children's reactions to divorce, divorce vs. conflict, and skills training.  A workbook Children in the Middle: Parents and Childrens Guidebook is provided to each parent.  Upon completion of the program, a certificate of attendance is sent to the court and becomes a part of the parents file. The program is provided by trained mental health professionals experienced in working with families. Each parent is responsible for registering for the program. This is a private fee based program and payment is required prior to participation in the program. Children are not allowed to attend the parenting course.  Grandparents, significant others, and step parents who may be making decisions that affect the child are encouraged to attend voluntarily but must pre-register and pay the class fee.  For further information contact 419 734 2942 ext. 0.
