ASSESSMENT
Assessment
service is the evaluation of an adult to determine the nature and extent of his/her misuse, abuse,  and/or dependency to alcohol and/or other drugs. The assessment includes a lab urinalysis. The assessment helps in determining needs, the type of services that will best meet the adult?s needs, the development of a plan of care and, when appropriate, referrals. This is a fee based service.  Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. CRISIS INTERVENTION
Crisis Intervention service is a face-to-face response to a crisis or emergency situation experienced by an adult and is to provide or assist the adult in obtaining those services necessary to stabilize the crisis situation. This is a fee based service.  Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. For further information:  Monday through Friday 8:00 am to 5:00 pm contact 419 734 2942 ext. 0.  After 5:00 pm, weekends and holidays contact 1 800 826 1306.

CASE MANAGEMENT
Case Management service includes activities to assist and support the adult in gaining access to needed medical, social, educational and other services essential to meeting basic human needs. Case management services may include interactions with family members, other individuals or entities. There is no fee for this service. INDIVIDUAL COUNSELING
Individual Counseling
service is the utilization of special skills including cognitive behavioral therapy and  motivational strategies by trained staff to assist an adult in achieving treatment objectives through the exploration of alcohol and other drug problems and/or dependency and their consequences, including an examination of attitudes and feelings, consideration of alternative solutions and decision making and/or discussing educational materials with regard to alcohol and other drug related problems. This is a fee based service.  Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. FAMILY COUNSELING
Family Counseling service
is the utilization of special skills by trained staff in sessions with the adult and their family members to address family and relationship issues related to alcohol and other drug abuse and/or dependency for the purpose of promoting recovery from dependency. This is a fee based service.  Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. URINALYSIS
Urinalysis service
is the testing of an individual?s urine specimen to detect the presence of drugs. This may be accomplished either through laboratory testing or urine dip screens. Laboratory testing is a fee based service.  Sliding fee scale (based on income and number of dependents) is available to residents of Erie and Ottawa Counties. A urinalysis screening is completed at the time of an assessment. There is a $10 supply charge for this service. GROUPS
Understanding Sobriety for Adults (USA) is a 8 week program meeting once a week for 90 minutes. The program offers education and personal growth for adults that have abused alcohol or drugs. The focus of the program, Interactive Journaling, exposes adults to the ?stages of change?, adopting strategies and techniques for positive lifestyle change, and  factual information on alcohol & drugs. Engaging adults through this program reinforces the idea that each individual is ultimately responsible for the choices they make. More than 10 million participants over the last 17 years have counted on this proven method of learning.. Our program encourages 12-Step participation. Random urinalysis screens are also a part of the program at no additional cost. Prior to participating in this program an assessment and a 1/2 hour session of individual counseling is required.
